After Martin does things to startle Frasier, Frasier puts on a clown outfit he has been saving for a phobic patient, and causes him to have heart attack. At this point a lot of issues and fears surface. Frasier, at times, seems incapable of admitting fault and when he does, he minimizes the results. There is a great scene with the elevator in the hospital waiting room.

This episode is an important one as it sees the character of Martin Crane being eased towards his final end point. This has been a bit rushed, but it's a nice journey for him, and those scenes between Mahoney and Wendie Mallick are well played.
Other than that it's another so-so episode, with the writers seeming to be struggling for anything else to do other than stick Kelsey Grammer in fancy dress again.
Interestingly the show also forgets its own continuity, with the fact that Niles had previously had heart problems being completely erased from everyone's minds it seems.
In order to get a sense of how the show is wrapping up it's an important episode, but quality wise it's not the best.
