For me the cards were laid on the table pretty early when our rather prideful doctor drives up to the Sedah (take a look at that name and run it backwards) sanitarium where the heating is broken and it's always too hot...
When we finally meet Eriq la Salle's 'Satan" we are well on our way to knowing what is happening.
Good film, well acted. A little obvious (yup, the good doc had landed in hell by my reckoning, right at the start) but worth watching.
