Monroe takes on a very special case when his principal assistant, Sally Fortune, collapses while performing surgery. It turns out that she not only has a serious brain aneurysm but also requires a heart valve replacement resulting from an infection. Operating on a colleague is stressful for everyone, all the more so when they learn Fortune and Andrew Mullery have started a relationship. Monroe and Bremner clash and the heart surgeon reconsiders her own relationship with Shepherd. Monroe helps his wife move into her own place but their son Nick admonishes his father for having given up any attempt at reconciliation with his mother.
