- A devious and psychotic student tries to frame a new girl at school for the death of a teenager at a party.
- The movie opens with Lexi Cooper who is the "queen bee" of her high school, and a callous prankster, as she and her friend Kaitlyn "swatted" their neighbors--calling the police with a phony distress call. She convinces Kaitlyn that no one got hurt and that if she tells anyone they are going down together.
Sierra Reynolds moves to Calabases from Aurora, Colorado, Lexi and Kaitlyn introduce themselves, with the former doing so as Alexis--adding that Sierra had to earn the right to call her Lexi. Lexi stated to Sierra that she had to earn her trust, while revealing that bad things have happened to people who have crossed her (in her mind).
On the following evening, Lexi invites Sierra to a party at her home, after which she points out the different cliques that have attended the party. The last student she is introduced to is Jake, who walks up to Kaitlyn and asks her out, after witnessing this Lexi decides to lace Jake's drink with GHB. Jake later feels the effects causing him to stumble and fall into the pool, where he drowns and despite the efforts of the Sierra and the other partygoers he dies, with his death labeled as an accident. Lexi who shocked by this orders Kaitlyn not to tell anyone about what she did.
Sierra is one of many who are interviewed by police, and she confesses that Jake may have been on drugs resulting in an autopsy performed on his corpse, which reveals that GHB was in his system. After learning this, Lexi lashes out at Sierra for speaking to the police, only for Lance to arrive and call out Lexi for being insecure, which angers Lexi.
Lexi is also one of many students interviewed by police after the report came out, with her mother, Jackie Cooper, present and lambasting her daughter, with her anger increasing after realizing that she'd be held responsible due to Lexi being under 18. Lexi was left to speak to the police alone, and afterwards, Sierra confronts her after hearing Lexi mention that Jake was drugged--leading Sierra to realize that Lexi was responsible for Jake's death. Their verbal confrontation results in Lexi accusing Sierra of killing Jake, causing Sierra to shove Lexi in response, after which Lance comes to Sierra's aid just in time to stop the confrontation from getting worse.
Lexi decides to set up Sierra, first by learning her locker combination by sneaking into the teacher's office and finds a list with the student's locker combinations. After opening her locker she plants a bottle of GHB inside it. Once Ms. Wilkins (the school's principal) receives an anonymous tip (from Lexi), she sets up a meeting with Sierra and her parents, Jeremy and Melissa Reynolds, and presents the GHB from Sierra's locker. Sierra denies ever owning the bottle and accuses Lexi of setting her up, but without evidence, Sierra ends up suspended pending the results of the investigation. Sierra furious leaves the meeting upset that no one, even her own parents don't believe in her innocence.
Sierra than goes over to talk to Kaitlyn knowing that she knows the truth. She arrives at her house and talks to her, but Kaitlyn denies everything and says that the GHB was found in her locker so it is obviously hers. Sierra shocked at the accusation, tells Kaitlyn that she and Lexi are much more deranged then she thought causing Kaitlyn to shut door in her face.
After Sierra's confrontation with Kaitlyn, Lexi meets with her and reminds her friend that Sierra killed Jake, with Kaitlyn disbelieving her accusation as they both know who was truly responsible. Kaitlyn suggests having a meeting between the three of them to talk things out, with Lexi agreeing before Kaitlyn phones Sierra. The three of them meet near a lake at night, with Lexi stating that all three of them would go to jail for Jake's death, claiming that Sierra knew what Lexi was doing (which wasn't true) and that she and Kaitlyn could have stopped her but didn't.
Lexi also mentions that she knows about Sierra's past and why she moved to their town, and how she had nearly killed her friend in her previous town. This causes them to get into a scuffle, which accidentally causes Sierra's cellphone to fall out of her pocket. Lexi and Kaitlyn notice that Sierra had been recording them, she tries to deny this but the evidence is clear. A furious Lexi grabs a bat from her car and strikes Sierra in the head as she tries to escape.
The blow however doesn't kill Sierra, but even after learning that she is alive, the evil Lexi orders Kaitlyn to help place Sierra in the back of her car, planning to kill Sierra and stage her death as a suicide. They force her to drink alcohol so that it is in her system and push her car down the lake with her in it, though unbeknownst to both of them, Sierra had survived, as the car didn't completely sink. She manages to escape from the car that had almost sunk all way to the bottom of the lake and makes it to shore before falling unconscious. Sierra's parents discover her missing and using a phone tracking app they find her cellphone and Sierra herself, and take her to the hospital.
Lexi is shown in her house in an argument with Jackie, who voices her exasperation with her having to constantly bail out her daughter and ordered her to move out of the house immediately. In response, Lexi lashes out at Jackie, stating that she had been taking care of her mother since the divorce, which occurred when Lexi was only 10 years old. Jackie repeats her order for Lexi to move out, Lexi appears to agree before shoving her mother over the railing, killing Jackie instantly.
Sierra is shown at the hospital finally regaining consciousness with her parents by her side. They reveal that the police are looking for Lexi after Sierra told them that Lexi had tried to kill her. The police arrive at Lexi's house and find her mother dead, with police believing that either she fell or was most likely pushed by Lexi.
Meanwhile Lance, Sierra's boyfriend has been trying to find evidence of Lexi's guilt. He manages to track down the dealer who sold the GHB to her. The dealer Tanner is confronted by Lance who reveals that Lexi had used the GHB she brought from him to kill Jake: his friend. Hearing this causes Tanner to regret selling the drugs to her in the first place, and gives him the money she paid him with. Lance visits Sierra at her house and tells her that he has gotten the evidence they need and that Lexi is going down for good.
Lexi meets up with Kaitlyn at school, and Kaitlyn terrified at her backs away down a staircase. Kaitlyn to Lexi's shock reveals Sierra survived and that the car hadn't sunk all the way and she managed to escape from the car as a result. Lexi accuses her of lying, before Kaitlyn reveals that her father who works at the hospital told her. She panics and tells her to say nothing, Kaitlyn says that It doesn't matter as it is all over, there is nothing they can do.
They struggle and Lexi attempts to kill Kaitlyn but is thwarted by the football team, as a last ditch effort Lexi sends the police (who were protecting Sierra) away so that she can enter the Reynolds house, doing so while carrying a gun. She calls out for Sierra, prompting Jeremy to appear and inform her that Sierra is not coming out. Sierra sneaks out of her room and hides, though Lexi nearly catches Sierra and fires a shot at Jeremy injuring his arm, and screams in anger over not being able to find Sierra.
Attempting to find Sierra, Lexi goes up to her bedroom upstairs, believing Sierra had escaped through an open window, and shouts that she will find Sierra, failing to realize that she had actually walked into a trap. Sierra suddenly appears behind her and shoves a shocked Lexi through the window, with Sierra and her mother wondering if she is alive or dead. They check on Jeremy who is injured but alive and call the police.
A few weeks later, Sierra is shown at a party thrown by her parents to celebrate their recovery. Lance and Sierra confirm their feelings for each other and celebrate with their friends.
In the last scene, Lexi is revealed to have survived and is shown being escorted into a women's prison. The final scene shows Lexi entering the prison in tears, with the other inmates preparing to "initiate" her; just as Lexi looks on with a fearful expression.
