- Contains more graphic violence than the previous films.
- In the earlier fight, an Evangelion unit shoots the other unit's head, destroying it (small amount of blood is seen), if you look clearly we can see the unit's jaw and tongue.
- In the later battle, another Eva decapitates a malfunction Evangelion unit, we can see blood sprays and a serpent like creatures emerges from its neck and head. There's a fight where one of the Evangelions turned into a beast-like like attitude and attacks its enemy, biting its neck and decapitating its head, we can see blood sprays from the neck and then shooting it rapidly, blood splatters. The enemy later regrows its head and biting the Beast Eva's head, crushing it and explodes. At the end of the film, we can see the mangled remains of the Beast Eva, a graphic sight.

- Shinji arrives in Rei's room, and he see her naked. We see her from behind for some seconds.
Violence & Gore
- In the beginning, Kaworu explains how everyone in the world they inhabit has been brutally murdered because of the "Shinji" the main character.
- Near the end, Kaworu decides to kill himself to save Shinji and the world. The collar in his neck explodes, with blood shown. Some could find it upsetting.
Frightening & Intense Scenes
- The post apocalyptic depictions may be disturbing. One sequence shows that the entire Tokyo-3 turned into a nothing but red wasteland, it is subtly implied that the citizens were killed and turned into an Evangelion-like creatures called The Failures of Infinity. In the town's landscape, we can see a teeth and an eye grows on the surface, also you can see many giant red cross during the scene. The Moon is also splattered with blood and red grids appears on its surface.
- During a sequence, a man reveals the truth of the Evangelion and then we can see a scene where a dozen of Rei's heads appears behind the man, this scene has creepy music and may be disturbing, later a scene shows a human shaped giant head with hollow eyes appears. A character later breaks down and we can hear voices appears out of nowhere, growing from a small chatter into shouting.
- The final battle takes place in a room full with giant human shaped skulls and pool of blood, we can see a blood stained giant cross inside the room, which may be disturbing. Lilith's headless body could also be disturbing, spears can be seen pierced its body, later it explodes alongside the aforementioned giant head, turning the room into a sea of blood (actually LCL, but the color is blood red). Before it explodes the giant head can be seen crying blood. The Fourth Impact scene could also be intense.
