When Monk and Natalie talk in the therapist's office, the position of Natalie's left arm changes repeatedly.
Lt. Disher and Capt. Stottlemeyer are seen playing a game of chess. Unfortunately, the chess board was incorrectly set up, so that the bottom-right square is black.
The chess game being played makes little sense. White has pieces all over the board, while Black only has two pieces remaining, which means Black should have already resigned. Lt. Disher moves his pawn diagonally, which is not a legal move because usually a pawn can only move one square forward.

Troy Kroger is clearly shown to be Dr. Kroger's only child. This contradicts Mr. Monk and the Election (2005), where Dr. Kroger is said to have a daughter. However, this discrepancy could be explained as being that something happened to Dr. Kroger's daughter, or the common practice in TV shows where characters are changed or even written out of existence over the course of a show.
After meeting the new psychiatrist Dr. Sorenson, Monk tries to block from his view Sorenson's half missing arm and covers his left eye as a way of blocking it from his view. However, it still would have been visible to him with his right eye. In order to completely block out Sorenson's arm, Monk would have had to have covered over half his face with something to block his right eye's field of vision.
When Madeline Kroger tells her son Troy that he has to accept Dr. Kroger as his father, Troy shouts "No!" However, Cody McMains' mouth does not open when the line is said.
