In "Tears of the Prophets," Federation, Klingon, and Romulan forces overtook Cardassia. Here, the Dominion still holds Cardassia and no mention is made of it having been captured or having been retaken by the Dominion and Cardassian forces. All that is said about the matter is that the Federation campaign to take the planet is going slowly, but the campaign was already shown to be successful several months prior.

When O'Brien goes to visit Worf in his quarters, the door is left open to allow the camera to see into the set. Even once the doorway has been cleared and the camera angle changes, the door is never heard making the distinct closing "swoosh" of a Star Trek door.
James Darren is obviously lip syncing during his performance of "You're Nobody 'til Somebody Loves You." His lip movements do not match the audio at numerous times throughout the song.
Worf joining Martok's crew leaves the Defiant without a commanding officer. Sisko, its regular commander, is on indefinite leave on Earth, and Jadzia, its regular second officer, is dead and was apparently not replaced in its command structure. Miles and Bashir joining the mission also deprives the Defiant and Deep Space Nine of their chief engineer and chief medical officer, respectively. That's a lot of important figures to go missing from a critical outpost and ship during wartime.
Colonel Kira, a member of the Bajoran militia, has been left in command of Deep Space Nine for 3 months. While the station is technically recognized as Bajoran property, the Federation have been acting as administrators for 6 years at this point. The station also serves as the base for the ninth fleet of the joint Starfleet/Klingon/Romulan task force. With Captain Sisko on indefinite leave, it is improbable that Starfleet would not assign a new commander to the facility, given its strategic importance. At the very least they would assign some more senior officers to bolster the command crew, as both Sisko and the station's second officer, Jadzia Dax, are no longer serving (Dax having died several months prior). Bajor technically has a non-aggression pact with the Dominion and cannot actively participate in the war effort, making Kira's continued command of the station even more suspect.
Sisko was well aware that the Dax symbiont had been taken to Trill for re-joining several months prior to these events. An unknown Trill turning up and showing familiarity with him should have at least aroused his suspicions that it was the new Dax host.
Worf believes Jadzia did not enter Suto'vo'qor because she "didn't die in glorious battle." She died facing Dukat, a supernaturally powered enemy, and drawing her weapon. That should qualify as a "glorious battle."
When Ezri says Dax's "seven other hosts" are with her, this contradicts her statement at the beginning of the episode where she lists six of them (Lela, Tobin, Emony, Audrid, Joran, and Curzon) and Sisko has to remind her of Torias. They don't mention Jadzia, but that could be implied. That makes eight other hosts, not seven. However, she appears to have also forgotten Verad; there are actually nine other hosts (Ezri is the tenth). Jadzia had frequently made a similar mistake, as she always stated up until she died that she was the eighth Dax host; she apparently didn't include Verad either.
O'Brien brings Worf a bottle of vintage 2301 bloodwine, which would be approximately 74 years old. Worf previously informed O'Brien in "Change of Heart" that he prefers his bloodwine "young and sweet."
When listing the things he and Worf discussed, O'Brien mentions "the proper way to eat gagh." Gagh is a traditional Klingon dish of live worms. O'Brien pronounces the word "gagh" with a "g" sound at the end, when it should be pronounced with a throaty "k" sound.
