The cassette tape that was put in to record the assassin's statement was white yet the type that Michael knight shows the villain of the episode just before he is arrested is black.
In the rooftop car park scene when Vlud is chasing Michael, one act ends with Michael conscious and alert, propped up on one elbow, staring at the approaching Vlud. The next act begins with Michael laid out straight on the tarmac, unconscious as Vlud bears down on him.
When Michael stops Vlud during the final confrontation, Vlud's car is several feet back from the cliff's edge. In the very next scene, his car is right up against the cliff's edge.
Throughout the episode, the two brackets at the front of Vlud's car which hold the ramming barrier alternate between being chrome and matte black.
The number plate on O'Mally's car is shown to be 958-OHF. After his car is destroyed by the train, KITT shows the license plate of the vehicle in the collision to be 1HTZ275, which is completely different.
In the very last scene KITT's door is already open (visible as the model walks off) before Michael walks around the car and the door opening sound is heard.
When Michael is chasing Vlud after O'Malley's murder, he asks KITT "Did you get a make on him?" and water is clearly leaking from KITT's roof. It's a sunny day outside, there's no reason for KITT to be leaking water.
Near the end of the episode, K.I.T.T.'s speedometer's L.E.D. bar-graph display is not working properly - when the digital (numerical) readout shows the car's speed as being at and then above 100 m.p.h., the 100 m.p.h. L.E.D. on the bar-graph still does not light up, nor does the one before it.
When Vlud rams Sam off the road in the opening scene, there's no-one inside the car.
Whenever Vlud flicks the switch to engage the bumper, it's clearly a model bumper attached to a dummy car.
When Mr. O'Malley's car is hit by the train it completely explodes. Unless there were explosives on board, a car would not explode into pieces when hit; it would simply be smashed.
During Michael's first visit to O&M Fashions the reflection of a sound department's reel to reel tape recorder with reels turning can be seen in the glass.
When KITT pulls up alongside Michael after saving him from Vlud, the entire camera crew is reflected and visible on KITT's door.
When Vlud is going through the car wash, there isn't anywhere near enough water to wash off all the paint shown on-screen.
