- This episode has six celebrity contestants competing for charity. The winner receives $50,000 for their charity and the others receive $10,000 for their charities. In the first stunt, contestants sit in a car at the end of a line of junkyard cars. As a monster truck drove over the line of cars, they have to find which of six keys started the car and drive over a finish line. If their car was crushed, they could still continue if the car would start. For the second stunt, contestants have to swim to the bottom of a water tank with over 1,000 snakes. They dig through the snakes and place as many of 100 yellow pucks as possible into a bucket in 90 seconds. Those left in the finale are hung over 200 feet above a dam and have to transfer flags from one corner of a Plexiglas box to the other. After 30 seconds, the floor would drop out and the contestants would bungee drop. The celebrity to transfer the most flags would be the Celebrity Fear Factor Champion.

For the first stunt, contestants have to hang by their ankles from trapeze hanging from a helicopter that flew over 20 flagpoles. The have to release as many flags as possible. In the second stunt, they are strapped down and their left arm would be pierced by 10 needles which would get progressively thicker. In the third, it's eating 4 ounces stinky cheese crawling with maggots. For the final stunt, contestants shimmy out to the end of horizontal pole 10 stories high, grab a flag, and clip it on to a rope hanging overhead. The fastest would win $50,000

The spinning starts and each contestant must unlock restraints at the seatbelt and steering wheel level, grab a flag from the back seat and swim with it to a buoy. Next was the eating of special 'sushi' dishes containing rice, maggots, rotten squid guts, red worms, super worms, earth worms, tarantulas, stink bugs, beetles, rotten fish guts, squid ink, and giant Madagascar hissing cockroaches. The final stunt was to walk out a six inch wide beam in the air and place flags as they go. A wall will continue to move over the beam and eventually push the contestant off the beam. The one that places the most flags wins.

- This was a special female models episode. In the first stunt, the models are chained into a weighted vest and dropped underwater. They have to undo three sets of clasps to get out of the vest and swim to the surface. For the second stunt, the models have to go through an obstacle course in a swamp and find three keys. They would then swim to shore and find out which of the keys unlocked a tunnel. They have to crawl inside and pull an 8-foot long, 400 pound alligator out by its tail. During the finale, the models navigate a series of ropes hanging underneath a helicopter. As they made their way from rope to rope, they would have to release flags attached to the ropes. The lady to release the most flags before falling into the water below would be the Models Fear Factor Champion and will 50K.

- The first stunt is a net jump as the contestants have to climb to the top of a 110 foot tower and jump twice their body length to a cargo net hanging across from the tower. Contestants who were able to hang on to the net for 5 seconds would advance to the next round. It's eating for stunt two as the ones left have to eat five earthworms and one super worm in a martini glass. Contestants who were able to do this in 4 minutes would advance to the finals. Lastly, the final contestants have to crawl through a pitch-black drainage system. The first pipe led to a wet and slimy culvert connected to several other pipes, but only one pipe besides the entry pipe did not have a dead end. The tunnel was rigged with infrared cameras so viewers could see the contestants in the dark tunnels. The contestant to reach the end of the exit tunnel fastest would win the $50,000.
